2011 Employer Benefit Plan & Health Care Cost Survey

Welcome to the 2011 Employer Benefit Plan & Health Care Cost Survey!

This survey is the ONLY survey of employee benefits design and costs in the Tri-State region. Your participation is greatly appreciated to ensure that the survey report is a reliable benchmarking tool for all employers in the area.

Please know that the information you provide will be kept strictly confidential – only aggregate results are reported.

We recognize the support of the Evansville-Area Human Resource Association in helping us communicate the survey out into the human resource community.

We have consolidated many of the survey items to make more efficient use of your time and added many enhancements. You will find a “Stop and Exit” button at the bottom of each page that will allow you to stop the survey as you need to. You will be able to resume from the last page completed as long as you are using the same computer and have “cookies” enabled. To help you gauge your progress, there is a progress bar at the top of each survey page. You are also allowed to move backward in the survey if you believe you need to change or correct a response. At the end of the survey you will be able to choose to receive an email with your responses for future references.

You can begin the survey today. As an incentive to complete the survey as early as possible, if you complete the survey by July 8, 2011 you will be entered into a drawing for your choice of a $200 gas card or a $200 donation to the charity of your choice. The survey will be closed on Friday, July 29, 2011.
